With the transition to ICD-10-CM, more specific code descriptions require more specific documentation from the physician.
Go to the Official Coding Guidelines: Section 1.C. Chapter-Specific Coding Guidelines in the very front of your ICD-10-CM book. Based upon these guidelines, what are some SPECIFIC items you would expect to be documented in a medical record? Provide at least one example using an ICD-10-CM code.

The code which are given here indicates Human Immunodeficiency Virus (HIV) Infections:
1) This Code only confirmed cases of HIV infection/illness. If the patient having HIV does not require documentation of positive serology or culture for HIV. The doctor or the supervisor’s diagnostic statement that the patient is HIV positive, or has an HIV-related illness is sufficient.
2) Numbering and ordering the HIV codes.
(a) The patient who is suspected as HIV positive are admitted, the doctor diagnosis should be B20
(b) If the patient is newly diagnosed for HIV conditions is unrelated to the sequencing decision.
If a patient is having the symtoms for HIV testing, code the signs and symptoms. An additional counseling code is Z71.7, Human immunodeficiency virus [HIV] counseling, may be used if counseling is provided during the encounter for the test.
If the test result of HIV testing is negative, use code Z71.7, Human immunodeficiency virus [HIV] counseling.
If the results are positive assign codes as appropriate.
