Explanation / Answer
ICD-9-CM is the International Classification of Diseases, Ninth Revision, Clinical Modification. It consists of six character alphanumeric codes for any diagnosis and procedure. It is developed to maintain uniformity in healthcare services. These codes are standardized and accepted by the U.S. healthcare system.
One of the outpatient coding guidelines is the additional diagnosis according to which the additional diagnosis should be listed secondary and the primary diagnosis should be the first listed diagnosis for the visit and service provided.
Current procedural terminology (CPT) is the set of codes used for reporting diagnosis, procedures and services in healthcare.
Symbol ‘+’ is used for add-on codes in the CPT coding book.
