Question

Anatomy question .

Written Lab, Ch 8 Fred was washing his windows on the second floor or his house. He slipped a fell from the ladder, landing hard on his stomach and shoulders. The impact of the fall was lessened to some extent when he landed first on his outstretched arms. He was suffering pain in his right wrist and pain in his right shoulder. In the ED workup, palpation of his right wrist just proximal to the base of the thumb produced severe pain as well as pain upon palpation of his right shoulder halfway between the shoulder itself and the neck. Two injuries were diagnosed and they were confirmed by Xray. 1. identify each injury [1 pt for each]; 2. describe what the xrays showed which confirmed each injury I 1 pt for each]; 3, Suppose Fred had landed instead upon the postero-lateral area of his right hip. He was able to move his right leg but he could not bear any weight on the right side when standing because of intense pain. An Xray in the ED confirmed the diagnosis. What was the diaghosis and what was seen on the Xray to confirm it. [I point for each answer] Total possibl e pts -8 mdicate

Explanation / Answer

Answers:

Scenario:

Fred slipped and fell from a ladder and landed on his stomach and shoulders, first on his outstretched arms – Developed pain both in his right wrist and in his right shoulder – Palpation of right wrist just proximal to the base of the thumb revealed severe pain + pain during palpation of his right shoulder halfway between the shoulder and the neck – X-rays revealed 2 injuries.

Explanation:

Outstretching of arms actually means extending the hands. During a fall, to combat balance and shock, a man usually put his hands at first on the ground followed by his body. Therefore, in this event, Mr. Fred had first put his right hand (might be his palm) leading to injury on his thumb.

Examination (palpation) just proximal to the base of the thumb actually represents palpation of thumb metacarpal bone. When this assessment indicates severe pain, it might reveal injury at his metacarpal thumb bone.

Given the history, noting that the patient has fallen on his stomach and the palpation of his right shoulder halfway between the shoulder and the neck revealing pain, it indicates right clavicle injury.

2. Describe what the X-rays showed to indicate the fracture.

3. If Fred had landed instead upon postero-lateral area of his right hip, he was unable to bear weight on his right side, but was unable to move his right leg. what is his diagnosis and what an X-ray reveal to confirm it?

Diagnosis is an injury to the right hip – at the iliac region.

An X-ray of hip confirm the diagnosis of right iliac fracture.

Explanation:

Firstly, the postero-lateral area of right hip region represent the iliac region and the patient might have encountered impact on this area. Secondly, Mr. Fred was able to move his leg which reveals that there is no injury to the leg region. Thirdly, as he was unable to bear weight in one side of the body (right side), the diagnosis might be injury to pelvic region. An X-ray of hip confirms this diagnosis.
