Cholera is a public health problem because cholera outbreak affects many people and it usually occurs as an epidemic. Cholera is a health issue because it causes severe dehydration due to loss of water and electrolytes. The cholera toxin keeps the transporter protein of sodium and chloride ions always open and allows the flow of ions out of the intestinal wall. Means the electrolyte concentration inside the intestine is decreased, which results in decreased tonicity (becomes hypotonic). The extra-intestinal space becomes hypertonic, which draws water along with the loss of chloride ions and other electrolytes out of the intestinal cells to maintain isotonicity. Cholera toxin acts through the protein disulfide isomerase (PDI), which is a non-competitive inhibitor of phosphodiesterase, thereby increasing the cellular cAMP levels and caused chloride ion efflux due to the activation of CFTR protein.
Cholera is still a health issue in several countries. The annual global incidence rate is 2.8 million and about 2.0 cases per 1000 people at risk of developing cholera.
