Assume that two nursing units are experiencing higher rates of patient falls than the other six nursing units in a rehabilitation hospital. In a minimum of 250 words, create a theoretical reason this is occurring. Provide a brief synopsis of what is occurring and the reason you have established it is occurring. In addition, using the plan-do-check-act process, describe how the issue will be addressed. Once your material is posted, be sure to review and comment on at least two of your peers postings. Your initial post to the given topic of discussion should contain a minimum of 2–3 peer-reviewed references.

Explanation / Answer

Every year, about thousands of patients are being reported regarding the falls that lead to severe injuries. The higher rates of falls in two nursing units than the other may be because of "high-risk patients" in these two units or due to staff negligence in these two units. For example, the nursing units with immobile patients may report the higher number of falls. In case of obstetrics (OBs), the fall injuries extend their hospital stay for another 6 days. On this scenario, implementation of the fall prevention programme is necessary to decrease the incidence of falls, thereby enhancing the safety of the patients.

The nurse has to consider the following interventions to prevent falls in this patient:

The post-fall debriefing huddle is found to be effective when it is combined with other fall prevention care plans, and it plays a key role in improving the quality of care. Debriefing allows identifying the risk factors of care and thus, the care plan can be modified appropriately.
