Ruby is 89 years’ old. She has fallen over in her home. The support worker, Joan, finds Ruby the next morning on the kitchen floor when she arrives at work at 9.00 am. Ruby was asleep, and appeared disorientated when she woke. She does not have any obvious physical injuries. After a cup of tea, Ruby is sitting in her living room and seems quite fine. She cannot remember anything unusual about the previous night and why she fell over.

Observations:
I visited ruby in her home today 9 am . She found to be felt down in the floor and asleep. Ruby is disoriented when she woke up and has no injury in her body. She had a cup of tea and found normal.she has forgotten everthing happened previous night and cannot recall the incident of fall . Consultation with a physician is necessary to rule out the reason for fall.
