Question
Chase, the community nutritionist, regularly works with home health care providers who care for some of the home-bound seniors in his community. His efforts to help them monitor the condition of their clients include using DETERMINE as a mnemonic device. He has given them the following mini case study and has asked them to write a response for each letter. Prepare a “key” for Chase to use to determine the success the providers had.
Case study:
Mrs. Jones, who was recently widowed after 52 years of marriage, has had type 2 diabetes for five years. It has not been well controlled and she is now experiencing some loss of vision and decreased kidney function. As a result, her doctor has added 5 new medications to the 4 she was already taking. She also takes some over-the-counter pain killers—either aspirin or ibuprofen—when her arthritis flares up. Mrs. Jones will soon be 81 years of age and she is glad that she still lives in the same house where she and Mr. Jones raised their three sons. Sadly, none of her children or grandchildren live close by. Before Mr. Jones died, they had a modest pension in addition to his social security. Now, she is facing a reduction in income since the pension will stop. Mrs. Jones has always enjoyed cooking and eating—though she doesn’t care for and doesn’t always follow her prescribed diet. Her weight has been steady for the past few years though she has started having more problems with her ill-fitting dentures. (10 points)
