Explanation / Answer
1. Identify 2 interventions the nurse will include on the survivorship plan of care that will address health promotion and management of comorbid conditions. Provide a rationale for each intervention
Though they have survived, they are still affected by defects in immune system. These defects would continue for the lifetime and hence, nursing interventions to promote proper health would be required. The nurse requires to teach the patient with life long strategies in which they would be able to avoid infections. Also, there might be other rules such as limiting alcohol, no smoking, eating good food as well as managing stress. Also, it would help if the patient is trying to build up his energy and strength by regular exercises. This could be created by the health care team.
Also, regular medical follow ups are required since, there may be other conditions such as thyroid cancer or hypothyroidism, that may take place. Pain management, physical therapy and nutritional planning regularly needs to be modified in order to take care of the patient.
2. Identify another member of the interprofessional team that could play a role in the cancer survivorship care for this patient. Explain the role of this interprofessional team member.
Another member who might be helpful is a nutritionist. Though, the nurse could provide an approximate idea of the food that should be consumed, but an exact diet chart would only be prepared by the nutritionist.
Another major role could be carried out by the occupational therapist, where the occupational therapist would help in pain management as well as emotional counseling.
