It is essential for the nurses to be aware of patients medications what they are taking and the effect of drugs in patients.
Nurses have to be aware of their home based medicines already they are taking before coming to the hospital and the current medicines
She has to talk with the pharmacist regarding the same inorder to avoid the double dosage or for the formulary of drugs
Educate regarding medicines to the patient and their family members are essential for the proper adherence to drug regimen
Communication of drug regimen during discharge palys an essential role in adherence and follow up after discharge is needed
Nurses has to assess for any new drugs ordered by physician or food to drug and drug to drug interactions to avoid adverse effects
