Explanation / Answer
Every organization have a certain values which guide their actions towards productivity and so is for nurses who actions are greatly guided by their values. For a successful attainment of mission and vision of the hospital, the values of both organization and nurses should go hand in hand which means the values of organization should be aligned with the values of nurses. The organizational values should be framed in such a way that it should include the values of nurses.
In the first stage of professional experience, the values of both the parties are aligned. But with the changes brought in the organizational values due to its restructuring, centralization of budget or resources, outdated and ineffective operating system, lack of proper communication, improper decisions leads to conflicts among nurses. When nurses perceive changes in the organizational values, they feel the patients care is compromised. It means the alignment between their values have lost and if its found to be severely compromised, nurses decide to quit the job. This effects the patients outcome as the quality of work gets effected. To explain it better I would cite an example from my own clinical experience.
The organizational value of the hospital in which I was employed was to be accountable to deliver cost effective quality care to patients with use of adequate supplies and competent staffs. My nursing value was to be accountable to deliver high quality care services within an adequate working environment to the patients. In the beginning these values was alligned with each other thus the indicators to evaluate the quality care should much improvements. I was extremely satisfied and was actively participating in patient care. But as the time advanced, the value of the hospital began to change. They stopped recruiting new staff and the ones they recruited was not competent enough even to carry easy task. The shortage of supplies laid a heavy burden on us. During night shifts, we had to keep begging in other units for supplies to carry out our works. Moreover, inadequate staffing placed huge workload on us. We were asked to manage 30 patients with two staffs including chronic rooms. The hospital also increased the duty hours. All these changes which occured in the organization values, broke the backbone of nurses. We become so frustrated with the management and it also created a huge conflict in us as we were unable to maintain our values, professional integrity and ethics. Those indicators which showed improvement in the beginning, began to show indications of poor performance by the staff and poor patient outcome as the quality of care was effected immensely. Thus, I decided to resign from my existing job.
The nurses including other health care providers have to face lot of challenges in the workplace which effect their output to greater extend. Many of these challenges result from improper communication between the members of the health team and with patients. The use of effective communication will not only solve these problem but will also foster a healthy collaboration between the various departments within and outside the hospital. The good communication technique helps to develop a good interpersonal relationship between patients and care providers which develops mutual trust and cooperation to adhere and participate in the treatment regimen. Its also important when a senoir authority deals with its subordinates. Many legal issues have been solved by use of effective communication. It helps one to understand the strengths and weakness of others and thereby enhances quality of work.
The needs of the system and the cultural differences of patients and its employees influences the outcome. The cultural beliefs of the patients should be considered while planning care for them. One should have respect towards these cultural differences and should be ready to accomodate these beliefs. Ex: according to the cultural practices of Muslims, the female patient should be attended by female physicians. If the care is planned according to their cultural beliefs then it will result in acceptance of the care and participation of the patients which ultimately effects the patient outcomes.
Accomodating the cultural differences while planning a health promotion and disease prevention activities in community is also important. Any intervention of such program will yield a positive outcome in the community only if there is participation of the community members. There readiness to learn and bring changes in their lifestyle and living pattern can result in health promotion and disease prevention. Therefore to enhance large community participation, the individual differences and needs should be identified and interventions should be incorperated considering these differences.
