Explanation / Answer

As per rules I am supposed tro answer only the first 4 sub-questions. However I will answer all sub parts of Q17 for you. The monthly premium is computed by dividing annual premium by 12. Of this $100 is payable by employer so, premium payable by employee= Monthly premium-100. Family premium = Monthly premium payable by employee+80

Accordingly the chart is presented below

Annual premium Monthly premium Monthly premium payable by employee Family premium payable by employee 1500.00 125.00 25.00 105.00 1250.00 104.17 4.17 84.17 990.00 82.50 -17.50 62.50 1900.00 158.33 58.33 138.33 2400.00 200.00 100.00 180.00 1875.00 156.25 56.25 136.25 1210.00 100.83 0.83 80.83 1050.00 87.50 -12.50 67.50
