Chapter 10: The Gastrointestinal System (GI): The gastrointestinal system is divided into two areas of study. Clearly this division is arbitrary and is based more on current medical practice than on scientific principles. Dentists and orthorhinolaryngologists, who also treat the diseases of the salivary glands, treat diseases of the mouth. Gastroenterologists are concerned with the diseases involving the remainder of the gastrointestinal tract, including the liver and the pancreas. The surgical lesions of the esophagus are in the domain of thoracic surgeons, whereas general surgeons operate on the abdominal parts of the gastrointestinal tract.

- A 40-year-old man had retrosternal pain that was most pronounced when lying down after meals. This was associated with regurgitation of gastric acid into his mouth. What is the cause of his pain? What is the most likely diagnosis?
Case #5:
-5. A 60-year-old man with a history of heavy smoking and alcohol consumption could not swallow meat. What should be done? What is the most likely diagnosis?

Explanation / Answer
CASE 4
The symptoms of the patient are RETROSTERNAL PAIN and REGURGITATION of food upon lying down. This is probably due to an anatomical lesion through which part of the stomach comes through diaphragm. The RETROSTERNAL PAIN is due to the acidic contents of the stomach going back into the OESOPHAGUS causing it's inflammation called oesophagitis.So the diagnosis in this case is HIATUS HERNIA
