Question

Case Study:

Kara invited the girls in her first grade class to her birthday party. Kara has a cute little bunny named Buttercup. Mrs. Jones, Kara’s mother tells Kara to “Make sure that none of your friends put their fingers in Buttercup’s cage because she might think that they are feeding her and accidently bite them!” Buttercup was placed in her enclosure and a sign that read, “DO NOT FEED THE BUNNY” was placed on the pet bunny’s enclosure.

When Kara’s friends arrived, she showed them her pet bunny and promptly told them not to feed her bunny. The group then made their way to the other side of the yard to play some games. Brooke, who was a big animal lover wanted to have a closer look at the bunny and “decided to pet it to feel how soft it was.” As she put her hand in the enclosure, the bunny promptly bit the end of her finger. “Ouch! That hurt!” she cried and then ran to join the rest of the children. She decided not to say anything to Kara or her mom because she did not want to get in trouble for not obeying the sign.

A few hours later, Brooke noticed that her finger was red and swollen. When her mother came to pick her up, her mother asked, “What happened to your finger Brooke?” Not wanting her mother to really know what happened, Brooke replied, “I don’t know, I think I bumped it really bad while I was playing.” When they got home, Brooke’s mother put some triple antibiotic ointment on the wound and then put a Band-Aid over the top. A few weeks later, Brooke’s finger was still somewhat red, warm and swollen. Brooke also complained about pain in her finger when trying to do her schoolwork. Mrs. Jones became even more concerned when Brooke began having fevers off and on over the past week.

She then called the pediatrician for an appointment. Following evaluation by the pediatrician, they were quickly referred to an orthopedic doctor who immediately ordered a finger x-ray and a MRI of Brooke’s arms and legs. The radiology report of the finger x-ray revealed a area of osteolysis over the dorsal aspect of the metaphysis of the distal phalanx. MRI results are still pending.
