A major factor in the pathogenesis of otitis media is dysfunction of the auditory (Eustachian) tubes. What is the function of the auditory tubes? at surgical procedures may be used to treat the fluid build-up in the middle ear? 3. What are possible complications to otitis media? 4. The patient reports relief (no more pain) and a liquid is leaking from the ear, is this a good thing?Explanation / Answer
1. Eustachian tubes are the connecting tube between nasopharynxa and middle ear.
They remain closed in normal situation however they may open during feeding and other forced respiratory maneuvers.
Chnage in altitude, Flight ascent or descent, diving all these can change the pressure in and around eustachian tube.
Major functions of eustachian tube
a> Maintaining a pressure equilibrium in and around Middle ear and nasopharynx
b> Mucus drainage from middle ear
Q2. Surgical care is preferred in case of Fluid build up in Middle ear evident by a bulging TM
Procedure name: Tympanocenetesis
What is done: By a sterile needle TM is punctured to drain out excess fluid from Middle ear
* Surgery is indicated once Antibiotic therapy is NOT satisfactory
Q3. complications to OM:
1> Unresolved infections may lead to chronic OM
2> Increased fluid accumulation, excesive bulging of TM can lead to Membrane rupture
3> Deafness: In case of chronic cases there is a chance of development of hearing loss
Q4. In case the patient suddenly reports a dramatic pain relief and leakage of liquid, It means Rupture of TM.
In case of uncomplicated OM, this rupture generraly heals after the control of infection.
* However in complicated cases Tympanoplasty may be required
