Case Study Investigation #14 A 79 year old female neighbor asks you to drive her to the doctor for a weekly examination. She lives alone, and her children are out of town. She claims that she is afraid to drive herself to the doctor because of “medical reasons.” At the doctor’s office you discover that she is still recovering from a fall that broke her hip several weeks ago. You think this is the reason that she is hesitant to drive. However, by overhearing the physician, you learn she cannot control her bladder function. Certain movements cause her to urinate without control, she often has to go several times through the night. On the drive home she admits her “situation” to you, saying that she never had “that problem” until after surgery for her broken hip. She does not understand the problem and is hoping you can explain her condition. You find out her medical exams come back normal except for the frequent urination. What is the cause of her urination problem? Did it happen after the fall or after the surgery? Determine the possible problems causing her condition.

Ans: Due to the bladder catheters urinary tract infection may occur which results in frequent urination wth burning sensation. Also, frequent urination is due to the removal of excessive fluids removal by body which is given during the surgery. and also if diabetic then must test blood sugar frequently after surgery so as to ensure that the cause of frequent urination frequency is not due to high sugar levels.
Since, the internal obturator muscle is connected to the levator ani muscle and this levator ani muscle is involved in supporting the pelvic organs. Since the short external rotators may have been atrophied due to hip joint dysfunction before surgery, if the strength of this muscle group recovers, support of the pelvic organs and urinary incontinence may be improved. The external rotation contracture of the hip joint and leg length which increased tension of the internal obturator muscle with which tension of the pelvic floor muscle also increased and improved urinary incontinence.
