Explanation / Answer

The video that I have selected is “2016 ExxonMobil Fire at Baton Rouge Refinery”.

a. The incident occurred at Exxon’s Giant Baton Rouge refinery in Louisiana. The incident occurred in the month of November in 2016. The exact date was 22nd November 2016.

b. The root cause that initiated the incident was that some of the pipe valves (around 3% of the total) had old design. In fact the valve that the workers were trying to open had a 30 year old design. Secondly the accepted practices within the refinery to open the valves lacked safety hazard analysis. This put the workers at greater risk.

c. CSB recommends that all operators should be adequately trained. What need to be implemented is to train operators as to how to remove inoperable gear box from the older models. Exxon did not have such training in place for its refinery operators. Thus CSB recommends that Exxon should establish accurate as well as comprehensive written procedures for those workers who are performing hazardous work or work that is potentially hazardous. Training should be provided to workers so that they are able to perform all anticipated job tasks in an optimal manner. Emphasis should be on hazard awareness.

d. Yes, the hazard was obvious before the incident. This was because workers at Exxon performed hazardous tasks without any guidelines from a comprehensive and detailed written procedure. With no emphasis and focus on hazard awareness a chemical incident was just waiting to happen.
