Microscopic Description:
Needle biopsies were received for histopathology. There is very little recognizable hepatic parenchyma. The samples consist primarily of a very dense, inflammation. There is associated bile duct hyperplasia and fibroplasia.
Microscopic Findings:
VERY SEVERE HEPATITIS WITH FIBROPLASIA AND BILE DUCT HYPERPLASIA
Comment:
This xxxxxxxxx is very severely diseased… Given the elevated globulins, viral infection is one possible cause. A cholangiohepatitis resulting from an ascending biliary bacterial infection is also a consideration.

To make a report simple to patient, we must write
The patient is suffering from severe liver disease. The disease is affecting the normal functioning of the liver . This may result from the excessive alcohol which he may take. Hyperplasia refers to increase in number of cells or tissue of the liver and that of bile duct. There is inflammation in the liver and there is obstruction in the bile duct due to accumulation of fibrous tissue in it. All these symptoms leads to inflammation of liver, known as hepatitis.
The most important function of liver is to make bile juice which help in the digestion of fat. This bile is travelled from liver and stored in gall bladder through bile duct. In the given situation, there is inflammation in liver which may be due to many reason like chronic alcoholism, viral or bacterial infection, drug induced or other. Cells of liver are degenerating and due to inflammation, fibrous tissue are accumulating causing blockage into the duct. As in this case, possible reason of liver inflammation could be viral infection which is indicated by elevated globulin level.
