The list of conditions this person has besides diabetes are as follows, hypertension, chronic back pain, lipid disorder and peripheral neuropathy, past history of ingrown toenails, patient had laser surgery on both eyes 7 years ago to correct retinopathies, patient had 2 smallest toes on left foot amputated 3 years ago to control gangrene, patient has not followed his proper diabetic diet in over six months, patient reports feeling run down for several months, patient has chronic congestion and sinusitis during allergy season, patient complains of dry mouth, patient complains of lightheadedness, patient reports mild tingling sensation and numbness in feet, patient has minor ulceration and cracking on feet, and the patient has reduced sensitivity to touch, pain, vibration, heat, cold, in feet and hands.
