Topic of systemic review: Reducing hospital readmissions

Research practice gap : "Oh, research is research, and practice is practice, and never the twain shall meet,", as quoted by Kipling is evident in the real world. A gap between research and practice occur due to various reasons, one being the researchers not thinking that the research done in a lab can be actually implemented- raising a deliberate gap. The second kind of gap being in the case where the research carried, at the time of implementation, is wrongly executed or messed up at the most critical steps. This is how the gap develops between research and practice.

Only with an improvement in the discharge planning and post discharge support infrastructure, there can be an improvement and reduction in hospital readmissions. This care can be in the form of increased patient - clinician communication followed by a proper follow up by the clinician. This can also be backed up by an in home facility or a skilled nursing facility or a home health agency and maybe a care manager, only with improvements in such care and facilities, can the readmissions be reduced.
