Diffusion - simple passing of molecule throught cell membrane. Membrane are made of lipids, non-polar molecules - it is non-polar itself. Glucose has 5 OH-groups in its structure - polarmolecule. When it try to diffuce it will be the same effect as if you tried to mix water and oil (no mixing, separation) - no passing. But cell need glucose, so it has to facilitate its transport by using membrane proteins which are also polar inside the cavity.
