Question
A 39-year-old male who has type II diabetes, but is otherwise healthy, has come into the clinic with a complaint of hyperhidrosis(excessive sweating), erectile dysfunction, and dysuria (patient complains that he cannot urinate normally). The patient submitted a blood sample for analysis in the week prior. The results of this test indicate that he has a fasting blood glucose concentration of 157 mg/dl; analysis of his urine sample at the time of examination indicate a high specific gravity and moderate traces of glucose and albumin. Upon asking the patient to come back to the examination room, the patient complains of slight dizziness. A quick examination reveals that the patient's resting heart rate is 92 bpm.
1.What condition do you suspect that the patient has (in addition to the diabetes)? Which specific symptoms lead you to that conclusion?
2.Based on the condition you have identified for this patient, what kind of drug would most likely be used to treat his urinary complaints? How would this drug help the patient's condition?
3.What type of drug would most likely be used to treat the patient's hyperhidrosis? How does this type of drug treat this condition?
Explanation / Answer
TYPE II DIABETES MELLITUS: It is a life threatening disease and life long disease which affects the way your body handles glucose a kind of sugar in your blood.
It is defined as the group of metabolic disorders in which there are high blood glucose levels for prolonged period is called Diabetes mellitus. Where as type II diabetes mellitus is due to insulin resistence , in this conditions the cells fail to produce insulin properly. the common cause of type II DM is excessive body weight and lack of enough excercise.
signs and syptoms: -Being thirsty -irritable -blurr vision -numness of hands and feet -wounds dont heal - recuurent infections -polyuria -polydipsia -polyphagia.
The NORMAL FASTING BLOOD SUGAR LEVEL IS 100mg/dl. if it is 100 -125 mg/dl is considerd as prediabetes. If it is more than 125mg/dl considered as high levels of blood glucose.
High blood glucose level on longer periods can damage and cause problems like --blood vessels and herat problems, kidney failure, nerves damage which trouble with digestion, feet, and sexual response, delayed wound healing, eye problems like catract, high blood pressure,
Based upon the scenario above the 39year old man who is having typeII DM with the syptoms of of excessesive sweating , polyuria, polydypsia, erectile dysfuction. And his fasting blood glucose level is 156mg/dl which is considered as high blood glucose level.the patient with high blood glucose level for longer periods can even develop high blood pressure in whiich your heart is to work harder to pump the blood.high BP can strain your heart and damage the blood vessels and can increase trhe risk of heart attack, or stroke.
HEART ATTACK SYMPTOMS- discomfort, heaviness, chest pain ,dizziness, excessive sweating, rapid or irregular heart beats.
1A)Based upon the scenario with specific symptoms of dizziness ,excesive sweating rapid or irregular heart rate i suspect in additon to type I DM the patient is having heart problem
2A)dysuria is defined as the painful urination due to infection in bladder, kdney, urethra, prostate. To treat the urinary complaints antibiotics are prescribed like pencillin, cotrimaxzole, amoxilln, norflaxcin.these drugs decrease the infection and can reduce the pain during urination
3A)for hyperhidrosis we prescribe antiprespitand drugs like IONTROPHORESIS,BOTULINUM TOXIN, and anticholinrgic drugs. iontrophoresis which will help to block the sweat coming on to the skin surface
