Diabetes mellitus is a group of diseases that affects the blood glucose level. Pancreas does not produces enough insulin for metabolis of glucose and hence glucose level increases.

Mr xyz 65 years old admitted in the medical ward in the view of diabetes . He has the complaint of weakness , unexplained weight gain since 2 months, iiritability, frequent urination and skin infection on right leg. He has the significant family history of having diabetes to his own brother.patient has the significant history of smoking habit.

Physical examination reveals:

Vitals signs: increased blood pressure, decreased pulse rate

General appearance: obese

Skin: sign of infection redness on right leg, wound is present, decreased wound healing

Mood : anxious, irritable

Vision: blurred

Weight: 75 kg

BMI: 32 kg / m2

Investigations:

1) random blood sugar test: >200 mg/dl

2) fasting blood sugar test: > 120 mg/dl

3) glycated haemoglobin ( A1C) test: > 6%

4) oral glucose tolerance test

Treatment:

1) insulin therapy 8 units

2) tablet metformin BD

Nursing management:

1) Assessment: General condition, vital signs, skin integrity, blood glucose level
