1. Prevention of Cholera:

Water control:

As water is the most important vehicle of transmission of cholera, all steps must be taken to provide properly treated water or otherwise safe water to the community for all purposes. Use of safe beverages. Avoid drinking beverages on street vendors.

Excreta Disposal:

When cholera appears in the community and taking into considerations of customs and practices of the population, explain the importance of handwashing with soap after defecation. Use latrines or other sanitation systems like a chemical toilet to the disposal of feces. Clean latrines and contaminated surfaces by use of house bleach. Washing should be away from drinking sources.

Food sanitation:

Since food may be an important vehicle for infection, steps should be taken to improve food sanitation. Health Education must stress the importance of eating cooked hot food and of proper individual food handling technique. Cooking utensils should be cleaned and dried after use.

Vaccination:

'Vaxchora' is a live oral vaccine for cholera is approved in the US. An epidemic and endemic of cholera should be reported regularly to the government. Two other non-live cholera vaccine such as Dukoral and shanchol are also available but they are incomplete protection.

2. Evidenced-based prevention Strategy:

The causative organism for cholera is Vibrio cholerae. Recent findings of using satellite remote on coastal regions to predict cholera outbreak. This system allows for timely prevention and spread of outbreaks of cholera. The best evidence-based prevention strategy is Haitian outbreak.

In the past 30 years, the surveillance data has revealed and case fatality rate of cholera has dropped and death due to cholera is rare in Bangladesh. This is achieved by early prediction and identification of community at risk. Whereas in Haiti, the inability to predict cholera exceeds the fatality rate to 6%.

The case fatality rates were high initially in Haiti, but later increased access to safe water brought down the case fatality rate. Two vaccines Dukoral and shanchol are produced but it will take a decade to vaccinate all the vulnerable people in Bangladesh. Evidence showed that implementation of safe water and sanitation, and hygienic measures in localities reduce the case in Haiti from 12% to 2%.

Water and proper sanitation regulations, vaccination in advance are essential. Epidemic warnings of cholera outbreak would be issued to the community. The cholera burden can be reduced by both predict and preventive measures.

3. (a). Cholera is a life-threatening condition but it can be easily preventable and treatable. Some of the prevention programs in the community are the verification of diagnosis, Notification, Early case finding, Establishment of treatment centers, sanitation measures, chemoprophylaxis, and vaccination.

It is achieved by early preparation of government with the help of district health offices. We initiate steps to use water purification tablets in all flooded areas. Flood shelters are protected by applying bleaching powder and germicides. We involved in massive campaigns and make awareness of the water-borne disease.
