Explanation / Answer

1.Pilot fatigue due to long hour flying time and time difference between flying station along with human own clock. Due to this, some time pilot wrongly read the data or does mistake in interpretation.

2.Language barrier is another issue along with ascent of the flight controller which can be wrongly interpreted by the pilot. Some time wrong information is shared by the Controller due to busy traffic and many aircraft is in the que for landing at different heights and so mistake can happen from Flight controller side as well.

3.As communication is parallely going to various flights and so it is possible to peak wrong information by the pilot of another flight.

4.Altimeter reading and scale can be different for different model and also the unit and lack of briefing or casual approach from the Captain can also lead to error.

5.Caliberation error of the altemeter or single map available only to pilot and so 1st officer may not have the same information and this can lead to lack of coordination between 1st officer and pilot.

6.Severe weather condition can also lead to error as either altemeter is not caliberated for extreme weather condition or failure happen in adjusting the height and picking signal.

7.Ergonomic and design of the gauge is aother reason of wrong data interpretation as different modelas and manufacturere has different design of the altemeter gauge and frequent chnage for the pilot to fly different aircraft can also lead to such error.
