A patient comes into the hospital with obvious symptoms of tertian malaria. The patient mentioned that they had malaria before when they lived abroad, but had been now living in Iowa for at least two years and had not been out of the country since then. The patient was upset because they now had to cancel a long-awaited trip to Africa and were unsure if they would ever be able to see the continent. How is this possible? What parasite or parasites might be responsible? Is it relevant that they had never travelled to Africa before?

Malaria was eliminated from USA in 1950. Hence it is not possible to get infection in USA although mosquito like Anophelese are found in USA.
The infection is possible in two conditions
1. When a person goes and come back from malaria infected country.
2. By a infected person who recently visited out side location into a malaria infected country.
In question the patient had not visited outside in last 2 year so it is only possible to get malaria infection from a infected person. The parasite plasmodium will be responsible for the disease.
Since the patient lived abroad may be some infected country so travelling Africa is irrelevant.
