The patient has been suffering from chronic renal failure so that decreased urine output is due to this renal failure caused at the time of circulatory shock. This is also causes decreased arterial pressure associated with low distolic pressure followed by hyperkalemia and low sodium. This chronic renal failure is also due to destruction of nephrons in kidneys followed by low GFR (glomerular filterate rate)
