B be the event that mixing error.

P(A) = 4.23% = 0.0423

P(B) = 3.4% = 0.034

P(A and B) = 2.03% = 0.023

And we have to find P(A or B).

By using addition rule,

P(A or B) = P(A) + P(B) - P(A and B)

= 0.0423 + 0.034 - 0.0203

= 0.056
