Question
Case Study #57 Case Presentation You are working in the college health clinic when a 20-year old female is brought in complaining of rapid onset fatigue in the muscles in her legs, with pins and needles feelings that come and go throughout the day and pain in her lower lumbar back. She has just recovered from an episode of mononucleosis and thought that it was a reoccurrence of "mono" until the pins and needles starting happening. You begin the physical exam and note a resting heart rate of 98 bpm, blood pressure of 136/88 and a respiration rate of 19 rpm. She also presents with hypotonic reflex tests (2+/4) bilaterally for her patellar tendon tap-reflex, absent Babinski test and weakness (3-/5) in manual muscle testing. You contact the Physical Therapy department at the college and they arrange for a nerve conduction test that indicates a conduction speed that is 85% normal. Based on your current understanding of Human Anatomy and Physiology, and with the use of correct medical/scientific terminology, generate a 2-3 paragraph response with a diagramed feedback loop describing the physiology of the case, that indicates the systems of the body that are impacted and the physiology of that system which was impacted, include in your response: What is abnormal and what is the most likely issue for the 20-year old female? What is the etiology (cause) for the onset of the issue? What other tests would need to be performed? What would be the most likely course of treatment? What is the probable outcome? Does she need to worry about progression and reoccurrence of issues? What could happen if the issue were to progress without treatment? In 1 paragraph, and using lay terminology and your understanding of human Anatomy and Physiology, walk your patient through the findings, conclusions, and possible treatment options.Explanation / Answer
This could be due to following reasons:
1.an active stress response and persistent stress cause muscle to be tighten which can cause pins and needles feeling in the extremities or any other body parts.
2.This also affects blood vessels to tighten
3.rapid change in breath rate,it could be shallow or quickly, hyper or hypo ventilation.
4.adverse effects in nerve system.
Since it is mentioned as mononucleosis: professional or doctor would suggest corticosteroids medication to reduce throat and tonsil swelling.
Diet and treatment include
1.lot of rest
2.stay hydrated
3.incase patient is non vegetarian, she could prefer warm chicken soup or vegetarian equivalent to the preferred diet.
The medical tests say that patient is prone to hypothyroidism, check cholesterol levels,or condition of inflamed stomach ,could be dry hair, sensation of feeling cold.
Take complete analysis of lipid profile, and patient is anemic, use proper medications .EBV test is preferred immediately.
