You are a hospital pharmacist working as part of a multi-disciplinary team on the stroke ward. Every morning you attend a multi-disciplinary meeting (MDM) to discuss the progress of your patients. The MDMs ensure that everyone is up to date and working together to achieve best outcomes to enhance a patient’s health outcome and optimise their functioning prior to discharge from the hospital. Members of your team include:

Doctors

Occupational therapist

Speech pathologist

Physiotherapist

Social Worker

Dietitian

   2. The Patient

Your current patient, Mr TF (80 years old), has had a stroke as a result of a blood clot that travelled to his brain. The clot was dissolved in the emergency department, before his admission to the stroke ward, but there appears to be some cell death in his brain that has affected his functioning. Mr TF is having trouble expressing his words and has lost mobility in the left side of his body. Some of the health professionals believe they will be able to improve his functioning but he will likely require a different approach to everyday tasks such as making coffee. Mr TF is worried about this as he lives alone at home and has no family close by. He does not know how he will cope with all these new medicines and ongoing doctors’ appointments.
