pH = pKa + log(base/acid)

6.4 = 6.09 + log(base/acid)

(base) = 2.042(acid)

(acid) + (base) = 0.22 M x 1 L = 0.22 mol

(acid) + 2.042(acid) = 0.22 mol

conjugate acid = 0.07 mol/1 = 0.07 M = 70 mM

conjugate base = 0.22 - 0.07 = 0.15 mol/1 = 0.15 M = 150 mM
