Question

Blood Banking Question:

1.You are performing an antibody identification test on a patient and determine that 10 of 11 cells are weakly reactive on the panel at the AHG phase, and the Auto Control is negative. You are considering that the antibody could be either an HTLA or a High-Frequency Antibody. Outline how you approach both possibilities, including all necessary testing.

What special considerations are given to blood products intended for special transfusion populations, including:    (12 points)

--neonates

--bone marrow transplant recipients

--individuals with impaired cardiovascular function

--individuals with hemolytic anemias

--emergency transfusions

--massive transfusions

Explanation / Answer

All the necessary testing to approach both the possibilities will be as follows:

1. Checking the previous records- This helps in identifying the nature of problems in antibody and exclude any new antibody.

2. No need to look for any kind of pattern. Cross out all the improbable options.

3. There can be reactions due to suspect antigens and can lead to the conclusion that some of the antigens were left uncrossed.

4. Follow rule of three. This means that there must be 3 ag positive samples that react positively with the sample of patient and 3 ag negative samples that react negatively with the sample of the patient.

5. Since it is given that the auto control is negative we can safely assume that there was no transfusion for around last 3 months and ag typing done is correct and valid.
