Ms. Haddad typically works a twelve-hour shift at the local small-town hospital. The current rotation is three days on and one day off. Last week she worked Sunday through Tuesday nights, had Wednesday night off, and then worked Thursday through Saturday evenings with Sunday evening off.

The hospital is chronically short staffed and Ms. Haddad frequently works double shifts, staying at the hospital for twenty-four hours at a time. Ms. Haddad frequently is the nurse asked to take the double shift as she is unmarried and does not have childcare responsibilities.

Ms. Haddad manages to spend time with her friends on the evenings that she is off, but she finds that there are many social activities that she has had to give up due to her irregular schedule. She often is free during the weekday and her friends are at work.

Current Problems Reported

Ms. Haddad complains or fatigue, but when she gets tired, she tries to sleep longer on her days off. If she is off on a weekend and she is spending time with her friends, she may not sleep for 48 hours at a time. She then has difficulty staying awake at work and has asked her doctor to prescribe some stimulant drugs to get her through some of the long shifts. She is also having trouble with mild depression and she feels isolated and lonely.

4. The hospital has been having trouble finding night staff and has decided to rotate all staff from eight days on day shift to eight days on night shift. Will this be better for Ms. Haddad, or would you predict either more or different problems?

5. What recommendation would you make to Ms. Haddad to improve her sleep and her waking performance?

1.Ms. Haddad can develop reduced immunity, she can be more prone to infection, irritation in nature and depression in case of sleep deprivation especially the REM phase of sleep deprivation.
